LIABILITY INJURY WAIVER, RELEASE & CONSENT AGREEMENT
I am familiar with the nature of soccer and indoor soccer
or the intended event of participation. I understand that participation in indoor soccer
or participation event can be dangerous and can possibly lead to serious and permanent bodily injury. Such bodily injury could
impair my ability to learn, to earn a living, to engage in other business, social and recreational activities and to generally enjoy life. I accept all risks of injury and death.
I accept that ARENA SOCCER CENTERS, INC., dba OFF THE WALL SOCCER, is only providing an opportunity to use an indoor athletic facility. In consideration of the
privilege to use this facility I agree to assume all risks and to release and hold harmless ARENA SOCCER CENTERS, INC., dba OFF THE WALL SOCCER, its staff, agents, owners, officers, property owners, league directors, officials, sponsors, and any others having an interest in the facility from all liability, negligence, causes of action, claims, demands and damages of every kind which may arise out of my participation in any activities at this facility.
I will ensure that I will familiarize myself with the
rules of the game
or intended event and of the facility and will, to the best of my ability, play under control and avoid injury to self and other persons using the facility.
I accept any and all risk as described and acknowledge by selecting "I agree" below.
Consent: I do hereby authorize Arena
Soccer Centers, Inc. and its assigns to utilize any and all photographs,
pictures or other likeness of me or anyone assigned guardianship to me,
as they deem appropriate in its promotional materials or team films.
